1. The facility is lovely, open and sunlit.
2. The staff has a never-ending supply of friendliness, warmth, humor, as well as "let's do this!" energy.
3. The best part for me was socializing with other folks there who needed help getting back on their feet as I did. We were all "on the same page" of working towards health even though we came from varied backgrounds and ages. It's important to get out of bed and out of your room - meeting others and sharing our wisdom and experience is vital to avoid the depression isolation can foster.

Excellent staff and facility. My dad ended up here after a fall, and we are so glad he did. The staff is friendly, caring and really connected with my dad. Very well run, recently remodeled, spotless facility. The bonus was that the food was very good, my dad loved it. He was there four weeks and improved each week with the PT, OT staff. All staff treated him kindly and with respect. A big thank you to our friends at SLO Transitional Care. The best care you could ask for.

San Luis Transitional Care was an amazing place to recuperate after surgery for a broken hip and femur. The facility is clean and lovely, makes you feel like you are at a fine resort. The entire staff, nursing, down to housekeeping was very friendly and kind. Physical therapy was twice daily and very good. The therapists really know their job. The food was good and I liked having choices. I was in no hurry to leave and am recommending SL Transitional Care to all who may need it.

Our mother needed to be transitioned after a week long hospital stay. We chose San Luis Transitional Care and we are glad we did. All staff members we interacted with were caring, professional, friendly and wanted to help. Since they had the availability we were able to have her stay for an extended period before she could be moved to an assisted living facility.

I have had four full joint replacements and was so grateful this wonderful facility helped me through challenging recoveries. They always follow the surgeon's orders, carefully recording and charting everything from the medications, progressive physical therapy, offer many thoughtful activities for those that want them, a terrific cook and impressive servers, even came around with a lovely afternoon/evening snack cart (even if some were too sick to partake), etc.... Both the day and night shifts are outstanding in countless ways. They were always kind and patient and respectful, encouraging and showing me how to walk again and coordinate again, how to dress myself before I went home, taught me how to use stairs, and to believe in myself again after major surgeries! Just wonderful staff that helped me through my fears and lack of confidence and pain...

I saw three former 1-star reviews (by people writing about their mothers who were patients) that are simply not true... there are call buttons at every toilet, every bed, and a nurse or aide come directly as often as needed 24/7. They do not give just Tylenol after surgery, unless it is specified by the Surgeon! My chart stated my latex allergy and they were all helpful and careful at all times. It may be an emotional time for some patients and their concerned family members? They understand that also...

This outstanding facility hires the most competent, caring, patient, lovely, kind, compassionate staff and I was blessed to be in their dedicated care. Thank you.

My mom came to this facility after having major back surgery. She has been here before, a few years ago for hip replacement surgery. She really likes the physical therapists and the food (that's the two stars), but several of the general nursing staff are not great and they are understaffed. They are not taking good care of my mom. They leave her for up to an hour on the toilet, or don't come at all so she wets herself in bed (happened once), and didn't listen to her when she said she has latex allergies, so she ended up with blisters from the bandages. The blistering rash itches like crazy so I bought my mom some cortisone cream, but the night nurse refused to put it on her. She's in hell. She called me crying saying "they broke me". And this is a woman that did full hip replacement surgery last time with nothing but tylenol as a pain killer. What does that tell you about the care she is receiving? This place needs to do a complete overhaul of their nursing staff and recognize that the people that come to a transitional care facility are trying to get better and live, and should be treated with respect and dignity.

I have had several family members and friends go through transitional care here. As a result I have spent a lot if time here, and interacted with numerous staff members. I have always found the facility clean, the staff friendly and helpful, and the patients happy. Much of a person’s perception about the care in a facility has to do with their own personality and the level of their compliance and acceptance of their situation. Take negative reviews with a grain of salt. This the transitional facility I would choose for myself.

Go up
